Increase capacity, identifying patients that need your attention

Asthma is a chronic respiratory disease requiring ongoing medical management. Currently, patients with asthma have a follow-up appointment approximately every 6 months. Most of the times, patients will be fine when they have their follow-up appointment. However, it does not mean that they can not have an acute episode in a short period of time. Thus, it would be better if patients could have their appointment when they need it, instead of the date that is scheduled by default.

What does Hilly do?

  1. Contacts patients periodically;
  2. Helps patients assessing an exacerbation;
  3. Warn health teams if an exacerbation is happening.
